Your First Visit
Welcome to Golden Foothills Oral, Facial, & Dental Implant Surgery! During your first visit, we will talk about your oral health and perform an examination. The consultation appointment is also an excellent opportunity to get to know our doctors and staff. After we discuss your goals for treatment, your oral surgeon will create a treatment plan unique to your needs.
We maintain open communication with our patients before, during, and after treatment. As our patient, you have access to an easy-to-use platform where you will receive personalized content unique to your treatment journey and can reach your provider directly with any questions.

What to Bring
We ask that you bring the following items with you to your first appointment:
- Referral slip and X-rays provided by your referring provider, if applicable
- List of current medications and medical conditions
- Medical and/or dental insurance information
- Method of payment
- Valid form of ID, such as a driver’s license
Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation appointment.
What to Expect
Your oral surgeon will evaluate your oral health and explain your diagnosis and treatment options. We will often take 3D scans to get a detailed look at your facial structures and detect any underlying conditions. In some cases, such as tooth extractions, we can perform surgery on the same day as the consultation.
If you have questions about your treatment, we recommend writing them down beforehand and bringing them with you. That way, we can address all of your concerns and help you feel more informed and confident about your treatment. Prior to your scheduled surgery, we will send you instructions to follow, as well as other information specific to your treatment.
3D Imaging
We use some of the most advanced CBCT scanning and imaging technology available in dentistry today to accurately diagnose and manage conditions, create detailed surgical guides, and more.
Insurance & Financial Information
It is our goal to deliver exceptional care at a reasonable cost. For your convenience, we accept many different payment methods. Our staff will explain payment options and are happy to answer questions regarding the insurance process. We will work with you to maximize your insurance reimbursement, provided we have the necessary information. Please remember that payment is due at the time of service unless other arrangements have been made in advance. You are responsible for all fees charged by our office, regardless of your insurance coverage. You can pay for your oral surgery treatment using any of the following payment methods:

Finance Your Oral Surgery With CareCredit®
Our office is an enrolled CareCredit provider. CareCredit is a credit card you can use to cover out-of-pocket expenses that are not covered by insurance. You can apply online and once approved, you can use your credit card at hundreds of thousands enrolled providers nationwide, including oral surgeons, chiropractors, pharmacies, labs, and more. You can select the repayment plan that fits your needs and choose short- or long-term options. Apply online today and when it’s time to pay for your treatment at our office, simply use your CareCredit card.
